The Surprising Link Between Your Hormones and Your Hay Fever
What Histamine Actually Does
Histamine is best known as the culprit behind classic allergy symptoms. It is the reason your eyes water, your nose runs, and your skin gets itchy when you come into contact with pollen or another allergen. Histamine is stored inside immune cells called mast cells, which sit in tissues throughout the body, particularly in the skin, gut, and airways. When a mast cell encounters something it identifies as a threat, such as pollen, it releases histamine and a handful of other inflammatory chemicals, and that release is what produces the symptoms we recognise as an allergic reaction.
What most people do not realise is that mast cells are also highly sensitive to hormones, particularly estrogen.
Estrogen and Mast Cells Have a Direct Relationship
Mast cells carry estrogen receptors on their surface, and research has shown that estrogen can bind to these receptors and trigger the same kind of calcium influx that leads to histamine release (Zaitsu et al., 2007). In practical terms, this means estrogen does not only influence reproductive tissue. It can directly activate the very cells responsible for allergy symptoms.
A well cited review by Bonds and Midoro-Horiuti (2013) summarises a growing body of research showing that estrogen receptors are present on numerous immune cells, and that estrogen tends to push the immune system toward a more allergic response. This helps explain a pattern researchers have observed for years, that asthma, allergic rhinitis, and other allergic conditions become noticeably more common in women after puberty, once estrogen levels rise.
Progesterone Plays the Opposite Role
Progesterone appears to have a calming effect on mast cells, helping to stabilise them and reduce the likelihood of histamine release (Zierau et al., 2012). Progesterone also supports the activity of an enzyme called diamine oxidase, or DAO, which is responsible for breaking histamine down once it has been released (Maintz et al., 2008). Healthy DAO activity means histamine gets cleared efficiently. Reduced DAO activity means histamine can build up and symptoms can linger longer than expected.
This is where the cyclical pattern many women notice starts to make sense. In a typical ovulatory cycle, estrogen rises toward ovulation, then progesterone rises afterward during the luteal phase. Many women report that their allergy symptoms intensify around ovulation, when estrogen is high and progesterone has not yet risen to balance it, then settle somewhat in the days that follow as progesterone increases. As progesterone falls again just before a period, some women notice their symptoms creeping back up.
Why Perimenopause Can Bring New or Worsening Allergies
For many women, seasonal allergies first appear, or get noticeably worse, during perimenopause. This is not a coincidence. Progesterone tends to decline earlier and more sharply than estrogen during this transition, so its calming, stabilising effect on mast cells fades while estrogen continues to fluctuate, sometimes surging to levels higher than in the regular cycle. The result can be an immune system that is more reactive and less buffered than it used to be, which shows up as new sensitivities, more intense hay fever, or allergic symptoms that seem to appear out of nowhere.
What This Means For You
Hormones are one part of a bigger picture. Pollen count, air quality, and your individual immune history all still matter enormously alongside them. If your symptoms have a pattern that tracks with your cycle, or if allergies have shown up for the first time during perimenopause, there is a real physiological reason behind it, and understanding it is often the first step to managing it well.
From a naturopathic perspective, supporting this picture usually involves looking at a few different angles together. This can include supporting healthy progesterone levels where appropriate, looking at gut health and DAO function since much of the enzyme responsible for clearing histamine is produced in the gut lining, and reducing your overall histamine load during allergy season through diet and lifestyle choices. It also means working with your practitioner to understand your own pattern, since the timing and intensity of symptoms can look different from one woman to the next.
